As a courtesy to you, our staff will submit insurance claims for your medical care, so long as the proper insurance information is available to us on your first visit. Please bring in your insurance ID card for your primary and secondary insurance to every visit.
You are responsible for any amount not paid by your insurance carrier. There are often co-payments, coinsurances, deductibles, and other charges not covered in full by insurance carriers. These amount are due and payable at the time of each service or treatment. For your convenience, we accept cash, personal check, money order, Mastercard, Visa, or Discover for payment.
If you do not have insurance, payment is due in full at the time of service and before your delivery date. We will work with you to design a payment schedule that fits your needs. Please contact our Billing Office at (563) 355-1853 to make financial arrangements.
